Our business office staff will verify your insurance coverage before your visit to ensure all information is up to date. Please bring your legal picture identification, your insurance card, and method of payment for any outstanding out-of-pocket expenses. If your insurance has recently changed, please allow extra time before your appointment to update your records.

As a courtesy, we file insurance claims on behalf of our patients. However, please note that you are ultimately responsible for all charges related to your procedure at BioSpine Tampa. Any questions or disputes regarding coverage should be addressed directly with your insurance company or employer.

You may receive separate bills from your surgeon, anesthesiologist, pathologist, or laboratory depending on the services provided during your procedure.

BioSpine Tampa, maintains its own billing policies, separate from your surgeon’s office. Your procedure will be billed to your insurance plan; however, you are responsible for any in-network or out-of-network deductibles, co-pays, and co-insurance as determined by your insurance carrier.

All co-pays and known balances are due on the day of service. Remaining balances are due within 90 days of service.

We accept personal checks, Visa, MasterCard, Discover, and American Express. If you are unable to make full payment on the day of surgery, please contact our billing office to discuss payment plan options.

If you do not have insurance coverage, payment in full is required on the day of your procedure. We accept personal checks, Visa, MasterCard, Discover, and American Express. For special circumstances, payment plans for non-cosmetic procedures may be arranged upon approval from our business office.

Our staff will provide an estimate of your out-of-pocket costs prior to surgery based on the information available from your insurance provider. These estimates are not final and may vary depending on your physician’s operative report, procedure time, and other factors.

You will be notified of your estimated responsibility by phone or letter before your procedure. Please contact our business office at 813-642-2546 if you would like to request an estimate of your facility fees.

For your convenience, we accept personal checks, Visa, MasterCard, Discover, and American Express. Unpaid balances over 90 days may be subject to collections.

Because several providers are involved in your surgical care, you may receive separate bills from:

• Your surgeon
• The anesthesiologist
• Pathology or laboratory services

Each provider maintains independent billing and payment policies.
